2. Meeting with the OHPC
   • Contact the OHPC Chair to schedule your meeting date.
   • Complete the OHPC Pre-Planning Board Design Review Application Form (PDF) and required submittals.
   • Submit six (6) complete OHPC Application Packets to the Land Use Office at least one week before your OHPC Meeting date. After you meet with the OHPC they will provide you with a copy of the meeting minutes which must be submitted, by you, as part of your Planning Board Application Packet. All questions, communications, or information you have for the OHPC should be addressed to the OHPC Chair.
